If you are the Parent/ Legal Guardian of a RiverKid who needs copies of medical records, please click on the link below and complete the form completely, to assist us in getting your medical records collected. Once we receive the completed form, medical records will be sent electronically to the email address on file, unless otherwise indicated.

If we require additional information or if you are requesting medical records to be sent to another entity, we may need to contact you before sending medical records.
If you know you have previously completed a Release/Exchange of Medical Information form, you can send an email directly to medical.records@riverkidstexas.com with details regarding the medical records you are requesting.
